Diabetes care at home

Diabetes is managed in a kitchen, not in an exam room, and that is where we manage it. What is in the fridge, whether the meter is being used, whether the insulin is stored properly and whether anyone has actually looked at the feet lately are the things that decide how this goes.

What we manage

Blood sugar targets that fit the person rather than a textbook, oral medication and insulin adjustment, A1c and kidney function testing drawn at your table, blood pressure and cholesterol, and the annual checks that quietly prevent the worst outcomes: feet, eyes and urine protein.

For older adults we also ask a question that gets skipped: whether the target is now too tight. Aggressive control that causes hypoglycaemia and falls is worse than a slightly higher number.

What the home tells us

Expired insulin in a door shelf where it freezes. A meter with no strips because the last prescription lapsed. Test results written on a calendar nobody brings to appointments. Food that makes the prescribed plan impossible. These are the real reasons control slips, and none of them are visible from an office.

Feet, and why we insist

A diabetic foot ulcer usually starts as something small that nobody saw, in a place the person cannot easily reach or see. We check feet at visits, look at the shoes, and get problems seen early rather than late.

Can you draw my A1c at home?

Yes. Blood draws happen at your table and results usually come back within a couple of business days. We call with them rather than leaving them in a portal.

Will you work with my endocrinologist?

Yes. We handle day to day management and coordinate with your specialist, and we make sure their changes and ours do not collide.

Can you help if I am struggling to afford insulin?

Tell us. There are almost always cheaper equivalents, patient assistance routes or plan changes worth trying, and skipping doses quietly is the worst option.
