Rate, rhythm and how you feel
We check your pulse and blood pressure properly at each visit, run a twelve lead EKG at home when it is needed, and ask about the symptoms that matter: breathlessness, palpitations, dizziness, tiredness that is new.
The anticoagulation conversation
Blood thinners reduce stroke risk substantially and raise bleeding risk. That trade-off deserves a real conversation, weighed against your own falls history, your kidney function and what matters to you. We have it in your living room, with your family there if you want them.
If you are on warfarin, INR can be checked at home. We also look for the practical things that quietly cause harm: an over the counter anti-inflammatory taken for a sore knee, or a supplement nobody mentioned.
Falls and blood thinners
Fear of falls is the most common reason anticoagulation gets stopped, often without anyone weighing it up. We assess the actual fall risk in your actual home and make the hazards smaller, so the decision is based on evidence rather than worry.
