Care after a hospital stay in Bergen County
The two weeks after a discharge are when readmission is most likely, and the follow up appointment is often weeks away. If a family member has just come home from Hackensack University Medical Center or Holy Name Medical Center in Teaneck, we can usually visit within two to three days, reconcile the discharge medication list against what is actually in the house and check that home health has really been arranged.
