Depression in later life is frequently mistaken for ageing, for dementia, or for an understandable reaction to loss. It is common, it is treatable, and leaving it alone is not kindness.

Why it gets missed

In older adults depression often shows up as physical complaints, poor sleep, appetite loss, unexplained pain or apparent memory trouble rather than as sadness. Someone who has stopped answering the phone and stopped cooking is telling you something, and it is worth asking about directly.

We also look for the causes hiding underneath: thyroid problems, vitamin deficiency, pain, a medication side effect, alcohol, hearing loss, or the isolation that follows losing the ability to drive.

Treatment that fits the person

Medication can help a great deal and is not the whole answer. It has to be weighed against everything else being taken, because the wrong choice worsens confusion and falls. We also talk about the practical side: getting out, seeing people, sleep, and referral for talking therapy where you want it.

No. Low mood is common in later life but it is not normal or inevitable, and treating it changes how someone lives day to day.

I do not want another tablet.

That is a legitimate position and we will not push. There is a lot to work with before medication, and if we do suggest one we will explain exactly why and what to watch for.
