Geriatric and dementia care at home

For a person with dementia, a waiting room is disorienting and an exam room is frightening. The same appointment at home, in a familiar chair, is often unremarkable.

Care built around the person

We assess cognition, mood, nutrition, continence, mobility and sleep as a whole rather than treating each with a separate prescription. A frequent outcome of a first visit is fewer medications, not more, because reducing an overlong medication list is often one of the most useful things a clinician can do for an older adult.

Support for the caregiver

The family member doing the work is part of the visit. We cover what to expect over the next year, how to handle sundowning and repeated questions, when a symptom is urgent and when it is not, and what respite and adult day options exist in your county.

Advance care planning

When you are ready, we sit down for the conversation about goals of care, POLST forms and healthcare proxies. Doing this early, calmly, at home, spares families the version of the conversation that happens at 2am in an emergency department.

Often yes. Without the trip, the waiting and the unfamiliar building, refusal rates drop sharply. We also allow extra time on first visits with patients who need it.

Yes, we see patients in private homes, independent and assisted living apartments and family members' homes across our service area.
