Medicare Annual Wellness Visits at home
Medicare Part B covers one Annual Wellness Visit every twelve months at no cost to you. We do it in your home, which is also the best possible place to assess fall risk.
What is included
The Annual Wellness Visit is a planning appointment rather than a head-to-toe physical. It covers a health risk assessment, a review of your medical and family history, a full list of your providers and medications, height, weight and blood pressure, a cognitive screening, a depression screening, a fall risk assessment and a written five to ten year schedule of the screenings you are due for.
Why the home setting matters
Fall risk assessed in a carpeted exam room tells you very little. Assessed in your actual hallway, on your actual stairs, with your actual bathmat, it tells you a great deal. We walk the route you take at 3am and flag the specific hazards.
Cost
There is no copay and no deductible for the Annual Wellness Visit under Original Medicare when the provider accepts assignment, which we do. If we treat a new problem during the same visit, that portion may be billed as a separate office visit, and we tell you before that happens.

Questions about medicare annual wellness visits
Is this the same as a Welcome to Medicare visit?
No. The Welcome to Medicare visit is a one time appointment in your first year of Part B. The Annual Wellness Visit is available every year after that.
Do I need to be an existing patient?
No. We are happy to do the wellness visit as a first appointment and you can decide afterwards whether to transfer your primary care to us.
